• Undertake and manage complex social work assessments and interventions, promoting independence, wellbeing, and safety.

• Develop personalised, outcome‑focused care and support plans in collaboration with individuals and their families.
• Liaise effectively with multi‑agency partners, including health, housing, education, and voluntary sector organisations.
• Represent Adult Services at case conferences, court proceedings, and multi‑disciplinary meetings.
• Maintain accurate case records and documentation in line with professional and local standards.
• Contribute to quality assurance, audits, and service development initiatives that enhance overall practice.

Qualified and registered Social Worker with Social Work England.

Significant post‑qualifying experience within adult social care, ideally in a local authority setting.

In‑depth knowledge of key legislation including the Care Act 2014 and Mental Capacity Act 2005.

Strong assessment, analytical, and decision‑making abilities.

Excellent leadership, mentoring, and communication skills.

Experience supervising students or junior social workers.

Background in court work or safeguarding investigations.

Familiarity with Hillingdon or Greater London services and frameworks.
