Clinical Practice
- Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being.
- Assess, plan, implement and evaluate individual treatment plans for patients with long-term conditions.
- Proactively identify, diagnose and manage patients at risk of developing long-term conditions.
- Work collaboratively with healthcare professionals to diagnose, monitor, manage and treat long-term conditions in line with national and local policies.
- Review medication for therapeutic effectiveness in accordance with evidence-based practice and national/practice protocols.
- Support patients in adhering to prescribed treatments.
- Provide information and advice on prescribed and over-the-counter medications, including side effects and interactions.
- Prioritise and manage complex, urgent or emergency situations, initiating emergency care where required.
- Support health promotion and self-care strategies.
- Provide cervical smear services and manage female health needs.
- Implement and participate in adult and childhood vaccination and immunisation programmes.
- Provide travel health advice and administer travel vaccinations as appropriate.
- Deliver opportunistic wound care.
Communication
- Demonstrate sensitive communication styles to ensure informed consent.
- Communicate effectively with patients and carers, adapting to diverse cultural and communication needs.
- Anticipate and overcome barriers to communication.
- Maintain effective communication within the practice and with external stakeholders.
- Act as an advocate for patients and colleagues.
