Clinical Practice

  • Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being.
  • Assess, plan, implement and evaluate individual treatment plans for patients with long-term conditions.
  • Proactively identify, diagnose and manage patients at risk of developing long-term conditions.
  • Work collaboratively with healthcare professionals to diagnose, monitor, manage and treat long-term conditions in line with national and local policies.
  • Review medication for therapeutic effectiveness in accordance with evidence-based practice and national/practice protocols.
  • Support patients in adhering to prescribed treatments.
  • Provide information and advice on prescribed and over-the-counter medications, including side effects and interactions.
  • Prioritise and manage complex, urgent or emergency situations, initiating emergency care where required.
  • Support health promotion and self-care strategies.
  • Provide cervical smear services and manage female health needs.
  • Implement and participate in adult and childhood vaccination and immunisation programmes.
  • Provide travel health advice and administer travel vaccinations as appropriate.
  • Deliver opportunistic wound care.

Communication

  • Demonstrate sensitive communication styles to ensure informed consent.
  • Communicate effectively with patients and carers, adapting to diverse cultural and communication needs.
  • Anticipate and overcome barriers to communication.
  • Maintain effective communication within the practice and with external stakeholders.
  • Act as an advocate for patients and colleagues.
