Practice Nurse

Portland Medical Centre, part of Bourne Health Partnership,is seeking a proactive and enthusiastic Practice Nurse to join the healthcare team, with a focus on chronic disease management and patient care. • Main Duties of the Role

The post holder is responsible for delivering general practice nursing services to the whole practice population. The role focuses on evidence-based care for patients with long-term conditions and preventative health care. As an autonomous practitioner, the nurse is responsible for clinical decision-making and the delivery of safe, effective care. The nurse will work collaboratively within the general practice team, support policy implementation, and provide nurse leadership where required.

Clinical Practice

• Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being.
• Assess, plan, implement and evaluate individual treatment plans for patients with long-term conditions.
• Proactively identify, diagnose and manage patients at risk of developing long-term conditions.
• Work collaboratively with healthcare professionals to diagnose, monitor, manage and treat long-term conditions in line with national and local policies.
• Review medication for therapeutic effectiveness in accordance with evidence-based practice and national/practice protocols.
• Support patients in adhering to prescribed treatments.
• Provide information and advice on prescribed and over-the-counter medications, including side effects and interactions.
• Prioritise and manage complex, urgent or emergency situations, initiating emergency care where required.
• Support health promotion and self-care strategies.
• Provide cervical smear services and manage female health needs.
• Implement and participate in adult and childhood vaccination and immunisation programmes.
• Provide travel health advice and administer travel vaccinations as appropriate.
• Deliver opportunistic wound care.

Communication

• Demonstrate sensitive communication styles to ensure informed consent.
• Communicate effectively with patients and carers, adapting to diverse cultural and communication needs.
• Anticipate and overcome barriers to communication.
• Maintain effective communication within the practice and with external stakeholders.
