Overview
The post holder will work as a Clinical Team Leader (Occupational Therapist/ Social Worker/ Registered Mental Health Nurse) in the Kensington, Chelsea and Westminster Forensic Community Team providing brief clinical and social care intervention for service users. To triage referrals and co-ordinate duty assessments. Be responsible for the supervision of staff & their case load. Facilitate appropriate clinical practice of the team including CPN's, OT's, Administrative staff and AMHPS.
Draw up the monthly rotas. Develop and maintain close professional relationships with GPs, Community Mental Health Hubs, Specialist Community Forensic Services and other professionals. Ensure staff practice in a safe, appropriate and professional way with service users and their carers. Attend professional practice forums. Organise and investigate Safeguarding alerts in line with local Safeguarding policies. Chair clinical and team meetings as appropriate.
Provide senior support to the Team Manager. To assist the team to be compliant with KPI and monitor electronic data. To take a lead role in Dual Diagnosis within the Team.
- Work as Clinical Team Leader in the Forensic Community Team with multidisciplinary staff and service users.
- Triage referrals and coordinate duty assessments.
- Supervise staff and their caseload; facilitate clinical practice across the team (CPNs, OTs, Administrative staff and AMHPs).

- Ensure safe, appropriate and professional practice with service users and carers.
- Attend professional practice forums and organise/investigate safeguarding alerts per local policies.
- Chair clinical and team meetings as appropriate and provide senior support to the Team Manager.
- Assist the team in achieving KPI targets and monitor electronic data; lead in Dual Diagnosis within the team.
- Collaborate with partner teams and provider agencies to refer service users needing long-term support.
- Attend regular management and senior team meetings; deputise as necessary.
- Ensure staff maintain accurate and contemporaneous electronic notes and input data as required.
- Undertake holistic assessment of the mental health needs of adults in the community.
- Identify vulnerable service users in Primary Care and deploy resources within existing budgets.
- Carry out needs assessment of individuals presenting to the duty service using FACS criteria.
- Promote self-care and recognise the role of individuals, families and carers as co-producers of health.
- Identify vulnerable adults and provide programmes of support to protect and promote health and wellbeing, intervening when safety or welfare is at risk.
- Ensure CNWL Trust and LA protocols are adhered to by team members.
- Cover for absences of senior colleagues as required by service needs.
- Contribute to Clinical Governance, including Target Setting for Teams and Clinical Audit; participate as a key member of the Borough Service to ensure collaborative and innovative practice.
