Overview

The post holder will be part of the NCL Red Cell Community Team supporting the implementation of evidence-based care pathways and rehabilitation services for patients with Sickle Cell, Thalassaemia and Rare Inherited Anaemias in NCL and help further develop the Red Cell Community service. The post holder will provide first-class and evidence-based patient care and promote health through patient empowerment, health education and self-management in the hospital, patients' homes and at Camden & Islington Sickle Cell & Thalassaemia Centre.

Responsibilities
  • Provide support as a Red Cell Community key worker to a caseload of patients through regular home visits and telephone contact to improve quality of care and to support admission avoidance.
  • Provide specialised physiotherapy to patients who are case managed by other members of the team.
  • Work in close collaboration with the Whittington Health Haematology consultants, the Community Team, GPs and community services to provide a comprehensive and seamless programme of care for suitable patients.
  • Attend regular multidisciplinary team meetings including the hospital-based team meeting to review patient management, new referrals, and discharges; and, team meetings to share learning, reflective practice, and education.
  • Work as a practitioner assessing patients in their own homes, health centres and other community settings. The post holder will work flexibly and in close liaison with the hospital Haematology team to ensure continuity of service delivery.
