Position: Rehabilitation Assistant - (Integrated Care Assistant)
Location: City of Westminster

Overview

We work with patients and their families/carers to support their rehabilitation journey resulting in a safe discharge home. The successful candidates will be required to support the clinicians in the safe delivery of this care. The role will require the successful candidate to work as part of the Inpatient therapy team for Lily and Poppy wards at Meadow Court. The induction and entry terms are described below.

Responsibilities
  • Carrying out therapy plans as per the qualified OT/PT recommendations to help patients achieve their goals before returning home.
  • Organising and prioritising own delegated workload.
  • Adhering to organisational knowledge relating to Trust protocols and procedures, particularly administration of medicine and moving and handling.
  • Providing accurate records of information required by the Trust for audit purposes.
  • Ensuring effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.
  • Planning and organising own delegated work.
  • Identifying and reporting any changes in patient care needs or changes in physical or emotional well-being to the MDT leads.
  • Acting as the patient’s advocate and, by providing information and support, facilitating the patient’s own choices with regard to personal care, promoting independence and self-care as appropriate.
  • Maintaining, updating and developing personal, professional knowledge and skills to achieve high standards of patient care at all times.
  • Undertaking specific nursing and therapy tasks following completion of theoretical training and acquisition of competency.
  • Maintaining confidentiality in all dealings with patients and their records.
  • Offering support and teaching patients, relatives and other carers appropriate skills, imparting knowledge to promote positive health.
  • Having knowledge of and the ability to effectively use available resources.
  • Participating in the support of student nurses and new staff in the locality.
  • Undertaking training that leads to new skills to develop services.
  • Reflecting on practice regularly and planning personal development to achieve growth.
  • Accessing supervision from qualified team members to enhance practice.
  • Participating in internal rotations as per service requirements.
Induction, terms and conditions

Starting with NELFT, new starters will be welcomed and inducted into the Trust. All new starters join on the first Monday of each month and undertake a comprehensive induction of up to two weeks, including mandatory training, systems training and equipment allocation. This may include meetings with the executive team, senior managers and attendance at drop-in sessions focusing on engagement, health and wellbeing and key processes.

Induction will be held at the head office in Rainham, Essex.

High-Cost Area Supplement – Outer London 15% (minimum £4,714; maximum £5,941). This post may require visa sponsorship considerations; this position will not be supported for visa sponsorship if it does not meet minimum requirements. Full-time position: 1 x Full Time (37.5 hours per week, 9am–5pm). Band 3 rehabilitation assistant.
