Overview
Pivotal Role in Quality:
This isn't just clinical delivery, it's about facilitation, mentorship, and policy implementation. You'll be an essential link, working collaboratively to embed best practice and Coordinated Care across primary care, social services and care homes.
Expert Autonomy:
While supported by the Band 7 Specialist Nurse, you'll work with a moderate degree of autonomy, utilising your clinical judgement and discretion to provide direct advice and mentorship to front-line staff caring for dying patients.
Strategic and Clinical Development:
This post offers an immediate chance to develop your clinical leadership and strategic skills. You'll be instrumental in managing a patient caseload to facilitate best practice and collecting data to support key performance indicators (KPIs) for End of Life Care.
Do you possess exceptional communication skills and are driven to ensure every patient experiences dignified, high-quality End of Life Care? We are seeking a highly motivated and experienced clinician to join our End of Life Care Team in Merton as a Community End of Life Care Nurse. This is a unique opportunity to shape quality palliative care across our community.
- Significant experience and passion for palliative or end of life care.
- A proven ability to communicate clearly, succinctly, and with genuine empathy.
- The commitment to working autonomously and driving patient-centred, evidence-based care.
- We want to hear from you. This is your chance to make a profound difference at the most critical time for patients and their families.
