Overview

The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies, and locality teams to deliver excellent standards of patient care for those requiring support. The post holder will undertake unplanned care community nursing duties involving delivery of nursing services and holistic assessments of patients’ health needs using a person-centred approach. The service operates through pre-rostered shift patterns spanning 8am – 10pm, seven days per week, including weekends and bank holidays.

Responsibilities
  • Provide skilled nursing care under the supervision of the unplanned care advanced nurses and paramedics.
  • Coordinate care for housebound patients to avoid unnecessary hospital admission or long-term care placement.
  • Facilitate self-care for patients with long-term conditions and participate in health promotion and innovative activities and projects that enhance patient service delivery.
