This is an ongoing, full-time opportunity for experienced Social Workers passionate about making a real difference within the vibrant community of Lambeth. Join the Learning Disability Team specialising in supporting adults with profound and multiple learning disabilities, learning disabilities, and autism. This role provides the perfect balance of hybrid working, with approximately half of the week based in our central Brixton office near major transport links.

What you will do:

• Conduct Care Act Assessments and Reviews to ensure the needs of individuals are adequately addressed.
• Develop Care and Support Plans tailored to the specific requirements of adults within the learning disability spectrum.
• Participate actively in Safeguarding Adults procedures to protect vulnerable individuals.
• Apply your expertise in mental capacity and best interests decision-making, with managerial support for Court of Protection cases.
• Immerse yourself in the diverse Lambeth community, working closely with vibrant multicultural groups.
