• Coordinate care for complex cases under the Care Programme Approach (CPA)
• Develop co‑produced care plans with service users
• Conduct comprehensive risk assessments and manage risks effectively
• Ensure safeguarding practices are upheld
• Perform holistic assessments of mental health and social care needs
• Conduct Care Act assessments and maintain regular reviews
• Engage with carers and wider support networks
• Foster partnership working with other statutory and voluntary organisations
• Liaise with inpatient units for a cohesive approach to care
• Keep accurate records and write detailed reports

• Registered with Social Work England
• Degree or equivalent in Social Work
• Experience working with adults with psychosis and complex needs
• Proficiency in Care Act assessments and reviews
• Knowledge of key legislation, including Safeguarding, Mental Health Act, Care Act, and Mental Capacity Act
