Greensand Surgery is looking for a self motivated advanced nurse practitioner to join the team on a full-time (37.5 hours a week) or part-time basis. We would consider a job share for the right candidates. Applicants will be needed to provide a combination of minor illness and long-term conditions care including diabetes, respiratory, CHD and hypertension.
Main duties of the jobThe primary responsibility of this role will be to provide more complex input than that currently provided by the practice nursing staff, duties will include:
- Diagnosing and treating patients presenting with a range of acute and chronic conditions, making any necessary referrals and/or investigation in the appropriate manner.
- Implementing and evaluating individual specialised treatment plans for patients.
- Providing advanced, specialist nursing care to patients as required, in accordance with clinical based evidence, NICE and the NSF.
Greensand Surgery is a semi-rural, GMS, practice in mid-Bedfordshire with just over 10,000 patients. We are a dynamic, hard-working team whose aim is to provide high quality patient centred care. The partnership is supportive and cohesive and we have a full complement of enthusiastic team members. We provide a comprehensive range of medical services for our patients as well as meeting targets for the new contract.
The practice consists of three partners and four salaried GPs, a dedicated nursing team, clinical pharmacist, and admin staff.

Person Specification Qualifications- Current NMC registration with an active PIN.
- Experience of managing long-term conditions specifically asthma, CHD, HF, hypertension and diabetes.
- Experience of working in primary care is essential.
- Have an understanding of QOF and enhanced service management.
- Willingness to undertake training and education.
- Willingness to improve and develop the patient experience and patient care pathway.
- Experience of using Microsoft software.
