Overview
Responsible for the assessment, planning, implementation, and evaluation of patient care in the outpatient setting for behavioral health home care. Responsible for case coordination of patients and insurance precertification/utilization review. Patients served include adult and geriatric populations. The St. Luke’s C.A.R.E. delivery model (Continuity, Accountability, Relationship-based, Evidence-based) reflects a commitment to compassionate, competent care delivered by an interdisciplinary team. Care is provided through a therapeutic relationship with the patient and family.
A spirit of inquiry is encouraged and supported, utilizing small tests of change and formal research efforts when evidence is unclear. Evidence-driven protocols are employed to ensure reliable and consistent care.
