Overview

As a full‑time Care Navigator in Linn County, your role will be to provide care coordination and supportive services to patients in the area.
Key Duties- Review the medical and mental health assessments of all referred/assigned patients and assure the necessary connection and follow up with providers.
- Meet individually with patients throughout their treatment. Develop work plans with the patient to address remove obstacles as they receive treatment and record progress. Collaborate with the clinical staff to identify needs for the patients.
- Develop and maintain strong working relationships with the various community providers and resources in the region area served.
- Provide support for patients needing assistance with child care, housing, benefit applications, job search and other stabilization support services. Assist in scheduling appointments/make referrals to community resources as needed.
- Document all interactions in the electronic records system and maintain records to track referrals and impact of making referrals to community resources to report patient participation in resources where a referral was made as required by the program or grant assigned.
- Promote the treatment programs and collect materials to present to community partners.
- Complete required training including certification in data programs, case management, SOAR.
- Participate in planning and information meetings with other Care Navigators.
- Maintain strict confidentiality, in accordance with HIPAA and 42
CFR.
Qualified candidates will have a bachelor’s degree in a human services field and one year of related work experience. This position requires a valid driver's license and the ability to pass an extensive background check.
Salary and BenefitsASAC offers a competitive compensation with a starting wage of $22.71
. Additional compensation is available for candidates with extensive experience.
