Position: RN Home Health - Cedar City, UT
This opportunity involves providing essential home healthcare services to adult and pediatric patients and their families, delivering expert nursing care within their home environment. You will work autonomously, utilizing comprehensive assessment, planning, implementation, and evaluation skills to ensure the highest quality of care aligned with professional nursing standards and regulatory requirements.

Key Responsibilities:

• Conduct thorough assessments of patients' physical, social, psychological, cognitive, communicative, and developmental status.
• Collaborate closely with patients, families, physicians, and interdisciplinary team members to develop and regularly update individualized care plans.
• Educate patients and families to prepare them for ongoing care and support beyond the current treatment episode.
• Coordinate and direct the interdisciplinary care team, ensuring patient-centered care is delivered efficiently.
• Manage time, resources, and supplies effectively to prioritize patient needs.
• Evaluate patient responses to care interventions, adjust care plans accordingly, and ensure ongoing documentation meets legal and ethical standards.
• Maintain compliance with all hospice/home health policies, legal, and regulatory standards.
• Electronically transfer patient files to the main office at least twice daily to ensure seamless communication and continuity of care.
