American Traveler is hiring a travel RN for a Home Health position in Cedar City, UT, requiring at least 1 year of home health, hospice, or acute care experience and an active UT or compact RN license.

Job Details

• Home health setting serving adult and pediatric patients in their home environments,

• Caseload includes both home health and hospice patients,

• Uses Epic for charting,

• Day shift, 8-hour shifts, Monday through Friday with weekend and holiday rotation,

• 40 hours per week, 5 days per week,

• Must be able to commute to the office at least twice daily to electronically transfer patient files, or have reliable internet access to do so remotely,

Job Requirements

• Active UT or compact RN license required - no pending licenses accepted,

• Current AHA BLS certification required; must not expire prior to assignment start,

• Minimum 1 year of RN experience in home health, hospice, or acute care,

• ASN or Nursing Diploma required; BSN preferred,

• Valid driver's license with insured, reliable personal transportation and an acceptable driving record required,

• Basic computer skills and Epic EMR experience required,

• Ability to independently transfer patients and navigate varied home environments including stairs, uneven surfaces, and cluttered spaces,

• Ability to lift, carry, push, and pull equipment and supplies of varying weights,

• DOB and last 4 digits of SSN required for consideration,

Additional Information

• Provides autonomous, evidence-based nursing care in patients' homes, including assessment, care planning, implementation, and evaluation for home health and hospice patients,

• Collaborates with interdisciplinary team members including physicians, non-nursing disciplines, and support staff to develop and revise individualized care plans,

• Electronically transfers patient files to the office at least twice daily,

• Coordinates patient education, referrals, and follow-up planning for continued care needs,

• Candidates who have been permanent employees of an Intermountain facility must have been separated for at least one year to be eligible,
