Job Title: Registered Nurse (RN) – Homecare
Location: Cedar City, Utah 84721

Shift: Days (8:30 AM – 5:00 PM)
Schedule: Monday–Friday with weekend/holiday rotation
Duration: 13 Weeks with possible extension
Hours: 40 Hours/Week

Job Summary:

The Registered Nurse (RN) – Homecare is a professional caregiver responsible for assessing, planning, implementing, and evaluating care for Hospice/Home Health adult and pediatric patients in their home setting. The RN delivers high-quality, evidence-based nursing care in an environment focused on healing, autonomy, and patient-centered outcomes.

Working independently in the home, the RN utilizes the nursing process, technology, and interdisciplinary collaboration to ensure safe and effective care in compliance with legal, regulatory, and professional standards.

Roles & Responsibilities:

Assessment:

  • Perform comprehensive physical, social, psychological, cognitive, communicative, and developmental assessments as appropriate.
  • Gather and evaluate information from patients, families, physicians, and interdisciplinary team members.

Planning:

  • Develop individualized care plans using assessment findings and critical thinking skills.
  • Collaborate with patients, families, physicians, and team members to establish goals of care.
  • Prepare patients and families for ongoing support needs, including education, referrals, and follow-up care.

Implementation:

  • Provide and/or delegate nursing care consistent with the established plan of care and professional licensure.
  • Coordinate and direct interdisciplinary care through effective delegation and collaboration.
  • Prioritize and manage time, supplies, and resources efficiently to optimize patient care delivery.

Evaluation:

  • Monitor and evaluate patient responses to interventions and progress toward goals.
  • Revise care plans as necessary based on clinical findings and patient outcomes.

Professional Practice:

  • Maintain compliance with regulatory standards and scope of practice guidelines.
  • Ensure accurate, timely documentation of patient care.
  • Participate in professional development and support the growth of team members.
  • Electronically transfer patient documentation to the office at least twice daily.
Required Skills:
  • Strong verbal and written communication skills.
  • Basic computer proficiency.
  • Ability to work independently in a home setting.
  • Ability to safely transfer patients and lift/move equipment.
  • Reliable transportation with ability to commute and electronically transfer patient files twice daily.
Physical Requirements:
  • Frequent standing, walking, bending, lifting, and transferring patients.
  • Ability to navigate stairs and uneven surfaces.
  • Manual dexterity for documentation, medication preparation, and equipment operation.
  • Ability to drive safely and travel between patient homes.
Required Education:
  • Associate Degree in Nursing (ASN) or Nursing Diploma.
  • Bachelor of Science in Nursing (BSN). Preferred
Required Certifications & Licensure:
  • Current RN License in the state of practice.
  • Current AHA - BLS Certification.

Preferred Experience:
  • Minimum two (2) years of RN experience.
  • Experience in Acute Care, Home Health, or Hospice settings preferred.
