Sutter West Bay Medical Group (SWBMG) is seeking a full‑time Nurse Practitioner to join our Cardiology team at 1580 Valencia Street, San Francisco, California.
About the OpportunityThe NP will manage follow‑up visits for established patients, support transitional care efforts, and provide timely post‑discharge visits for heart‑failure patients. A key part of this role is educating patients on cardiovascular conditions, treatment plans, and lifestyle modifications.
Key Responsibilities- Conduct follow‑up visits for established cardiology patients
- Support transitional care management through post‑discharge visits
- Provide patient education and promote medication adherence
- Collaborate with cardiologists and care teams to ensure continuity of care
- Document patient encounters in Epic
- Help reduce patient wait times and improve access to cardiology care
- Work in a collaborative, patient‑focused environment
- Play a key role in transitional care and patient education

- Graduate of an accredited Nurse Practitioner (NP) program
- Current California NP license
- DEA Controlled Substance Registration Certificate (CSRC)
- Current BLS and ACLS certification
- Experience in cardiology or internal medicine preferred; new graduates with a strong interest are welcome to apply
