Employment type: Full-time
Salary: $120,000 - $135,000 per year plus $10,000 annual bonus. Base pay range: $120,000 - $135,000.

- Perform an evaluation in the home and develop a care plan, and work with PTAs to follow the care plan.
- Help patients with mobility, movement, and regaining independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure quality care.
- Evaluate and report patients’ progress to MD.

- Mileage/Vehicle Reimbursement – $0.70 per mile between visits.

- All candidates required to complete a computer literacy and competency assessment prior to an in‑person interview.
- Current unrestricted CA Physical Therapist License.
- Current BLS/CPR Certification.
- Valid CA Driver’s License.
- Knowledge of local community resources.
- Understanding of individual development and human behavior in relation to illness/injury and culture’s influence on health care.
- Knowledge of state and federal home‑care regulatory guidelines preferred.
- Experience in home health care highly preferred.
- Experience with high acuity patients a plus.
- Experience working with an interdisciplinary team highly desired.
- Case management fundamentals highly desired.
- Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized.
