Salary: $120,000 - $135,000 per year plus $10,000 annual bonus. Base pay range: $120,000 - $135,000.

• Perform an evaluation in the home and develop a care plan, and work with PTAs to follow the care plan.
• Help patients with mobility, movement, and regaining independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to MD.

• Mileage/Vehicle Reimbursement – $0.70 per mile between visits.

• All candidates required to complete a computer literacy and competency assessment prior to an in‑person interview.

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License.
• Knowledge of local community resources.
• Understanding of individual development and human behavior in relation to illness/injury and culture’s influence on health care.
• Knowledge of state and federal home‑care regulatory guidelines preferred.
• Experience in home health care highly preferred.
• Experience with high acuity patients a plus.
• Experience working with an interdisciplinary team highly desired.
• Case management fundamentals highly desired.
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized.
