Schedule: PRN – Days shift
Location: 2350 MEADOWS BLVD, Castle Rock, 80109
Plans, implements, and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; links to community resources.
Key Responsibilities- Assess patient’s physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records & clinical data.
- Evaluate patient’s response and intervene to ensure optimal outcomes.
- Develop and implement patient plan of care and observe outcomes in collaboration with the interdisciplinary team.
- Demonstrate competency in a variety of therapeutic/diagnostic interventions.
- Master of Social Work (MSW)
- Current Basic Life Support (BLS) certification from American Heart Association
