Field Nurse Practitioner​/Physician Assistant

Health Care Support is actively seeking a Field Nurse Practitioner / Physician Assistant to fill an opening with a growing Healthcare provider in Casa Grande, AZ!

• The Provider works in a field-based capacity completing visits to patient homes for Chronic Kidney Disease / Primary Care needs. Providers also have telehealth opportunity based on patient need and location
• Job duties include complete history and physical, creation of treatment plan, order of diagnostics and lab testing, prescribing of medications, routine team meetings, patient education, care plan assessments, documentation of services
• Schedule:
  
  Monday-Friday, 8am-5pm

• Licensed NP or PA in Arizona
• 2+ years of experience as an NP/PA
• Completion of clear Background check and drug testing
• Reliable transportation and a verifiable good driving record.

$140,000 - $150,000 per year + 10% Annual bonus
