We are seeking a dedicated and compassionate Primary Care Nurse Practitioner/Physician Assistant to join our healthcare team. Responsibilities

• Conduct thorough patient assessments, including medical history and physical examinations.
• Develop and implement individualized care plans based on patient needs.
• Perform diagnostic tests and interpret results to guide treatment decisions.
• Administer treatments, including IV insertion and tube feeding, as necessary.
• Manage patient care in various settings, including memory care facilities and teaching hospitals.
• Participate in discharge planning to ensure smooth transitions of care for patients.
• Maintain accurate medical records and documentation for all patient interactions.
• Engage in utilization review processes to optimize patient outcomes and resource use.
• Provide education and support to patients and their families regarding health management.

• Proficient in venipuncture techniques and other essential nursing procedures.

Pay: $90,000.00 - $99,999.00 per year
