Job SummaryThe Highland Rivers Behavioral Health Intensive Case Management (ICM) Team is a medium intensity community‑based program for adults with severe and persistent mental illnesses such as Bipolar Disorders, Schizoaffective Disorders, and other sub‑categories of thought disorders.
Responsibilities- Community based position.
- Provide transportation for the individuals.
- Must visit individuals' homes.
- Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the individual's Individual Recovery Plan (IRP).
- Partner with the individual and CORE services provider in the development of the IRP.
- Provide individual interventions to identify service needs; develop strategies and supportive interventions to avoid out‐of‐home placement or the need for more intensive services; assist individuals to increase social support networks to ameliorate life stresses resulting from the individual's condition; and coordinate rehabilitation services as specified in the individual's IRP.
- Serve as advocate with other agencies to garner needed resources for individuals.
- Provide assistance and support in crisis situations.
- Provide assessment, monitoring, and assistance to individuals.
- Provide direct service to individuals; complete all necessary paperwork according to policy; ensure individuals meet eligibility criteria or refer to appropriate provider in the community.
- Manage a caseload by ensuring all services are authorized, chart paperwork is complete and current, and individual charts are closed according to policy.
- See a caseload at the frequency defined in the service plan; document attempts to contact individuals and ISP reviews; complete monthly caseload reports; track appointments and case management.
- Meet weekly contact and productivity standards.
- Attend treatment team meetings as directed by Program Supervisor.
- Assist with service‑related compliance.
- Maintain confidentiality for all indirect/direct service in accordance with agency policies and HIPAA policies.
- Other responsibilities and tasks as may be assigned.
