Job Duties
- Conduct Home Health Nursing Assessments to determine nursing equipment needs, participant preferences and goals, and actively participate in IDT meetings to develop participant care plans
- Deliver and document home health nursing interventions as agreed upon in the participant’s care plans, including maintaining a healthy and safe environment, responding to physician orders promptly and accurately, and correctly administering medications and performing ordered tests and treatments
- Instruct the participant, family and caregivers regarding disease process, self‑care techniques, and prevention strategies related to medical conditions
- Track and monitor Home Health scheduling in collaboration with Home Health Manager or point of contact for scheduling
- Collaborate with the Welbe Health Physician and other IDT members to assist in smooth care transitions between settings (hospital, skilled nursing facility, home, etc.)
- Graduate of an accredited school of nursing
- Unencumbered RN license required
- Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population; home health agency experience preferred

$47.74–$63.02 USD per hour + bonus.
Actual pay will be adjusted based on experience and other qualifications.
