Position Responsibilities:Patient Care Coordination
:
Facilitate and coordinate patient care in collaboration with the multidisciplinary healthcare team based on physician orders.
Assessment & Planning
:
Perform patient assessments, develop nursing diagnoses, and create individualized care plans. Evaluate patient responses and adjust plans as needed.
Communication & Collaboration
:
Effectively communicate patient data to the healthcare team, make recommendations for patient interventions, and collaborate with other providers to deliver exceptional care.
Delegation & Supervision
:
Delegate nursing tasks based on patient needs, staff competencies, and regulatory standards to ensure safe and effective care.
Patient Education
:
Educate patients, families, and staff to promote wellness and healing. Provide a culturally sensitive and age-appropriate care plan.
Clinical Decision-Making
:
Utilize clinical judgment, inquiry, and collaboration to provide comprehensive, patient-centered care.
Licensure
:
Active, unencumbered RN license in the state of Texas.
Experience
:
Previous ER or acute care experience preferred.
Skills
:
Strong clinical, communication, and critical thinking skills. Ability to work effectively in a fast-paced, collaborative environment.
