Position: Registered Nurse Case Manager (Zionsville | Greenwood)

We are seeking compassionate, detail-oriented Hospice RN Case Managers to join our growing hospice team serving patients and families in the Greenwood and Zionsville areas. This is a field-based nursing role with an assigned territory, allowing you to provide high-quality, relationship-based care in patients’ homes and facilities.

At our hospice, we believe that every person deserves to be treated with dignity, comfort, and respect at the end of life. We are looking for a nurse who combines strong clinical judgment with a heart for service and the ability to support patients and families through one of life’s most meaningful transitions.

Key Responsibilities
  • Provide skilled nursing care to hospice patients in their homes, assisted living facilities, and nursing facilities
  • Manage an assigned caseload and coordinate care across the interdisciplinary team
  • Complete comprehensive assessments and develop individualized plans of care
  • Monitor symptoms, manage pain and comfort needs, and adjust care plans as appropriate
  • Educate patients and families about disease progression, medications, and comfort-focused care
  • Provide emotional support and guidance to patients and loved ones
  • Collaborate with physicians, social workers, chaplains, aides, and other care partners
  • Ensure accurate and timely documentation in the electronic medical record
  • Participate in on-call rotation as needed
Qualifications
  • Active Registered Nurse (RN) license in the state of Indiana
  • Valid driver’s license and reliable transportation
  • Ability to work independently in the field
  • Strong communication, organization, and time management skills
  • Compassionate, patient-centered approach to care
Preferred
  • Hospice, home health, or case management experience
  • Experience supporting patients with complex or chronic conditions
Compensation & Benefits
  • $88,000–$95,000 annual salary
