RN / Registered Nurse - Hospice Admissions

Location:
11550 N. Meridian St., Ste. 375, Carmel, IN 46032. Coverage area:
Carmel, IN.
Schedule:
Monday-Friday 10AM-7PM.
As a Hospice Admissions RN you will:
- Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
- Explain hospice services, discuss goals and care with the patient and families, answer questions, and obtain necessary consents and legal documents
- Obtain medical history, current medications, and symptoms
- Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
- Coordinate the start of services (nursing, aides, equipment, medications)
- Collaborate with interdisciplinary team
- Maintain accurate and timely documentation
- Medical, Dental, Vision insurance

- Graduate of an accredited nursing school with current licensure to practice in the state of operation
- One year of hospice nursing experience required; 3+ years preferred, or similar setting such as Long-Term Care, Intensive Care, or Emergency room will be considered in place of hospice nursing experience
- Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
