RN / Registered Nurse - Hospice Admissions

11550 N. Meridian St., Ste. 375, Carmel, IN 46032. Coverage area:
Carmel, IN.

Schedule:

Monday-Friday 10AM-7PM.

As a Hospice Admissions RN you will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

• Graduate of an accredited nursing school with current licensure to practice in the state of operation
• One year of hospice nursing experience required; 3+ years preferred, or similar setting such as Long-Term Care, Intensive Care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
