Specialist Childrens Disability Nurse - Disability Service

Do you have a passion in making a difference for children and young people with disabilities? An exciting opportunity has arisen for a part-time permanent Paediatric or Learning Disability Nurse within our Children's Disability Service, based in Carmarthenshire.

The main focus of this role is to enable children and young people to reach their full potential, have equal opportunities, choices and rights in their own community.

The successful applicant must have the ability to assess, plan, co‑ordinate and review children with disabilities, and their families, informing their health care and support.

The role incorporates being a source of health expertise in relation to disabled children, including child development, managing challenging behaviours, health promotion and prevention, and identifying early intervention opportunities.

You will need to have experience of working autonomously and managing a caseload, with the ability to support professionals, families, carers and communities with health information and advice in relation to children and young people with disabilities.

The post holder is also responsible for participating in the development and delivery of the Children's Disability service across Hywel Dda University Health Board.

• To address the health agenda for each child or young person.
• To manage a caseload of disabled children, having responsibility to identify and assess the health, social and educational needs of the child in partnership with other family members and other professionals, and to prepare and monitor individual care and support plans.
• To participate in the development and planning of services for children with disabilities.
• To offer specialist health advice and guidance to the wider multidisciplinary team.
• To undertake specialist clinics to support children and their families such as bowel and bladder and behaviour management.
• To promote independence and self‑care skills by supporting and teaching children and their parents/carers in collaboration with multi‑agency colleagues to address day‑to‑day issues such as managing their health condition, behaviours that challenge, toileting and sleep programmes.
• To prepare health reports/updates for Child Protection and Looked After children meetings.
• To contribute and participate in the assessments and reviews of services that have been provided to meet assessed needs.
• To work collaboratively with colleagues in the wider Children's Community Nursing Service to address the health agenda.

• Four main hospitals:
  Bronglais Hospital in Aberystwyth;
  Glangwili Hospital in Carmarthen;
  Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest.
• Five community hospitals:
  Amman Valley and Llandovery hospitals in Carmarthenshire;
  Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire.
• Two integrated care centres:
  Aberaeron and Cardigan in Ceredigion, and several other community settings.
• 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 99 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers.
• Numerous mental health and learning disabilities services.
