37.5 hours per week, 8am – 4pm

What you will do:

  • Collaborate with the Acute Discharge Planning Team to facilitate timely and efficient patient discharges.
  • Support early intervention efforts for highly vulnerable patients, ensuring their needs are addressed promptly and effectively.
  • Conduct comprehensive assessments and provide tailored therapeutic interventions.
  • Coordinate with multidisciplinary teams to develop and implement discharge plans that meet patients’ individual needs.
  • Monitor patients’ progress and adjust care plans as necessary to ensure optimal recovery.
  • Educate patients and their families about care plans, available resources, and self‑management techniques.
