Hywel Dda University Health Board are looking to recruit a full time Community Mental Health Practitioner with a qualification and registration as a RMN nurse, Occupational Therapist or Social Worker.
The successful applicant will join the Integrated Community Mental Health Team, based at Wellfield Resource Centre, Carmarthen. The post is permanent and will be full time - 37.5 hours per week. The hours will be worked in 7.5hr shifts over a 9am to 5pm shift pattern rota, Monday to Friday.

Our service operates 5 days a week and works through an Assessment & Recovery approach with people accessing Secondary Mental Health Services.
Main duties of the jobThe post holder will be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care for a defined clinical caseload including the role of a Care Co-ordinator within Care Treatment Planning under the Mental Health (Wales) Measure 2010.
The post holder will be required to work without direct supervision for their delegated caseload. The post holder will require effective communication skills and be able to develop positive working relationships between all healthcare professionals and statutory and voluntary agencies, maintaining good communication across all boundaries.
The ability to speak Welsh is desirable for this post;
English and/or Welsh speakers are equally welcome to apply.

Person Specification Qualifications and Knowledge- Professional Qualification/Registration from recognised body - i.e.: RMN, Occupational Therapist or Social Worker
- Post-basic education in the area of practice to postgraduate diploma level
- Wide range of experience within a service related setting
- Demonstrates achievements in developments of care standards/procedures guidelines
- Demonstrates effective team working
- Use of I.T. skills
- Demonstrates research and audit experience
