Looking for a Clinical Manager for our growing Hospice Team - Full Time
PositionClinical Manager – Hospice Team (Full Time)
Responsibilities- Plan and lead the initial implementation of all aspects of patient care and client satisfaction as it relates to the interdisciplinary team.
- Ensure that the team is in full compliance with the Plan of Care (POC) for each patient.
- Collaborate with the attending physician, Hospice House Medical Director, and other members of the interdisciplinary Hospice team.
- Participate in patient care conferences and supervise RNs, Home Health Aides, and LVNs.
- Provide oversight of care for patients diagnosed with a terminal illness and regularly review and ensure the following of the Plan of Care, including prescribed medical treatments, pain management, symptom control, assessments, and documentation according to the HNC Model of Care.
- Provide education and supportive care to patients and caregivers.
- Education: Graduate of an accredited school of nursing.
- Credentials: Current CA RN License. BSN preferred. Certification in Hospice and Palliative Nursing desired.
- Experience: Minimum of one year of experience as a professional nurse within the last 3 years or a Baccalaureate in Nursing. Two years in hospice, home health, or agency, primary care clinic or health facility, with at least one year in a supervisory or administrative capacity.
- Other: Valid driver’s license, reliable transportation and auto insurance.
