This RN Clinical Manager will plan, implement, and oversee all aspects of patient care and client satisfaction within the interdisciplinary team. The manager will ensure compliance with the plan of care for each patient, collaborate with the physician, Hospice House Medical Director, and other team members, participate in care conferences, and supervise RNs, Home Health Aides, and LVNs. The RN Manager will provide education and support to patients and caregivers.
Responsibilities• Plan and implement patient care plans and ensure compliance with the plan of care for each patient.
• Participate in patient care conferences and consult with interdisciplinary team members.
• Supervise RNs, Home Health Aides, and LVNs.
• Oversee pain and symptom management, assessments, and documentation according to the HNC Model of care.
• Provide education and supportive care to patients and caregivers.
Education: Graduate of an accredited nursing school; BSN preferred.
Credentials: Current CA RN license; certification in Hospice and Palliative Nursing desired.
Experience: Minimum of one year as a professional nurse within the last three years, or a Baccalaureate in Nursing; at least two years in hospice, home health, agency, primary care clinic or health facility; at least one year in a supervisory or administrative capacity.
Valid driver’s license, reliable transportation, and auto insurance.
