RN Case Manager

Compensation: $ - $ per year

Location: Carlsbad, CA

Qualifications:

• Experience in hospice care (ideally at least 1 year) with a good understanding of hospice rules and regulations
• Valid driver's license; mileage reimbursement offered (IRS compliant)

Responsibilities:

• Manage a caseload of 16-20 patients, conducting regular visits at least once every 14 days
• Assess patient needs, including more frequent visits (up to 3x/week) for patients with wounds or higher acuity, requiring critical thinking skills
• Travel within a service area of 25-30 miles surrounding the hospice center in Carlsbad, focusing on a specific region (north/south/east of Carlsbad) depending on the candidate's location
• Maintain productivity by managing 4-6 patients per day, with visits in both facilities and homes

Work Schedule:

• Hours from 8:30 am to 5:00 pm with some flexibility
• Rotating one weekend day per month with compensatory day off during the week
• Full-time position, 40 hours per week
