Support Coordinator

We are looking for an experienced, caring and motivated individual to lead a supported living service providing support for 7 tenants, delivering personalised support to people with learning disabilities and complex mental health needs living in their own homes. The supported living service provides 24 hour support and is set in one of Carlisle’s most desirable addresses, convenient for Stanwix, parkland and the city centre.

• Lead our supported living service to ensure the highest quality of life and promote independence for service users.
• Coordinate the staff team, including deployment and supervision, and work with families and other agencies.
• Direct support and participate in support planning with members of multi-disciplinary teams.
• Demonstrate strong leadership, planning and coordination skills.
• Work flexibly to include evenings and weekends as part of a rota to meet individual needs (37.5 hour per week post).

• Minimum of Level 3 Diploma in Health and Social Care (or equivalent).
• Candidates with a relevant social care degree or as a qualified social worker are welcome.
• Previous experience of working with people who are experiencing mental health problems and learning disabilities in a community or supported living setting.
• Skills in positive engagement, knowledge of positive behavioural support, and excellent verbal and written communication.
• Understanding of the recovery model, mental health diagnosis, and relevant legislation.
• Ability to communicate confidently and participate in support planning with multi-disciplinary teams.

• This is a 37.5 hour per week post with a flexible rota including evenings and weekends.
