Job Summary

To provide and maintain a high standard of nursing care for patients, as well as providing nursing assistance to other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced Band 6 or 7 (depending on experience) Nurse and any additional roles agreed between the nurse and the Kristin as appropriate, having regard to current training.

Responsibilities
  • Facilitating change by providing holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise.
  • Assessing and providing holistic assessment for long‑term conditions and contributing to an MDT approach for patient care.
  • Assessing problems presented opportunistically by patients, dealing with minor illness.
  • Able to document consultations according to NMC guidelines.
  • Ensuring awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child/adult abuse.
  • Performing venepuncture according to local guidelines.
  • Providing a_pa holistic approach to wound management and implementing wound care in line with current evidence‑based guidelines.
  • Suture removal.
  • Taking ECG gekommen.
  • Identifying and managing anaphylaxis according to current UK guidelines.
  • Performing cardiopulmonary resuscitation according to current UK guidelines.
  • Obtaining and documenting informed consent (either verbal or written).
  • Carrying out discussions and implementing personalised care plans.
  • Discussing and implementing DNACPR as necessary.
  • Ensuring infection control guidelines are maintained.
  • Monitoring and managing maintenance of stock and equipment to include refrigeration, steriliser and emergency equipment.
  • Maintaining confidentiality of information gained at work at all times.
  • Providing assessment, screening and treatment services and health education advice.
  • Organising and co‑ordinating the provision of nursing services for the practice, including delegation of duties to other staff as appropriate.
  • Providing nursing treatments to patients in participation with general practitioners or independently, meeting agreed protocols.
  • Providing general and specific health screenings to practice patients within agreed protocols with referral to general practitioners as necessary.
  • Advising patients on general health care and minor ailments with referral to GPs as necessary.
  • In‑participation with the Housebound Team Lead, ensuring the maintenance of equipment and stock levels relating to patient care.
  • Following appropriate training, undertaking.sf collection Hybrid despatch of pathological specimens and ensuring cleanliness of used equipment, performing requested investigatory procedures.
  • Participating in the administrative and professional responsibilities of the practice team.
  • Ensuring accurate notes of all consultations and treatments in patient notes on the clinical computer system.
  • Ensuring accurate completion of all necessary documentation associated with patient care and registration.
  • Collecting and maintaining statistical information required for regular and ad hoc reports.
  • Attending and participating in practice meetings as required.
  • Assisting in the formulation of practice philosophy, strategy and policy and developing appropriate protocols.
