Position Overview
We are looking for a nurse with a background in acute and/or rehabilitation care and with a specialist interest and knowledge of stroke. Candidates need to demonstrate enthusiasm and commitment to delivering and developing the Stroke service across the Health Board.
The Early Supported Discharge (ESD) Nurse plays a unique role within the ESD team, which includes physiotherapists, occupational therapists, speech and language therapists, psychologists and nurses. The nursing role involves visiting patients in their own homes to ensure they continue receiving nursing care while receiving stroke rehabilitation in the community.
Key Responsibilities- Provide on-going stroke nursing care to patients in the community, including blood pressure monitoring, pain assessment, mood and fatigue management, medication administration, and bladder and bowel care.
- Deliver secondary stroke prevention education, ensuring patients understand their diagnosis and ways to reduce future risk.
- Support patients who have suffered stroke, providing education and guidance.
- Work autonomously while engaging in service improvement initiatives.
- Identify suitable acute stroke ward patients for the ESD service and manage a caseload of home‑visiting patients.
- Attend multidisciplinary meetings to discuss patient progress and raise any issues to the patient’s GP or Stroke Consultant.
- Follow up stroke patients at 6 months post‑diagnosis, referring them to appropriate organisations as needed.
- Use the online system to record case notes and become familiar with the Sentinel Stroke National Audit Programme (SSNAP) quality improvement programme.
- Provide specialist educational support to a range of healthcare professionals, ensuring up‑to‑date stroke guidelines are followed.
- Background in acute and/or rehabilitation nursing with specialist interest and knowledge of stroke.
- Demonstrated enthusiasm and commitment to delivering and developing a stroke service.
- Ability to transport patients between sites and to conduct home visits.
- Basic knowledge of IT and experience recording notes online.
- Desirable:
Ability to speak Welsh (Welsh and/or English speakers are equally welcome). - For non‑UK or Republic of Ireland nationals:
Self‑assessment of suitability for a Health and Care or Skilled Worker visa; sponsorship may be required.
