Specialist OT Live Well Age Well

The Live Well team are looking to recruit a team leader to develop our existing project to support frail residents of Cardiff and the Vale to self‑manage and remain in the community healthier for longer. The main duties and responsibilities will be around managing the Live Well Age Well programme and supporting the MDT to deliver high value, high‑impact interventions. The post holder will work collaboratively with CRTs and community partners across the health board’s geographical area.

They will be managing a team of support staff who will help deliver this preventative programme.

The ability to speak Welsh is desirable for this post;
English and/or Welsh speakers are equally welcome to apply.

Main duties of the job

Person Specification Qualifications
  • Recognised qualification degree Occupational Therapy
  • HCPC Registration
  • Evidence of Masters level learning or relevant higher-level qualification and experience
  • Membership of the professional body
  • Masters level qualification or equivalent
Experience
  • Significant post registration clinical experience in frailty, falls and healthy ageing.
  • Evidence of clinical leadership and assisting in planning and developing of an existing service
  • Experience as working as part of a multidisciplinary team
  • Experience of teaching other disciplines
  • NHS Experience
  • Experience of working with early signs of cognitive decline
  • Provided advice and support to other practitioners to develop skills and knowledge
  • Involvement in developing professional practice, and transforming service delivery
  • Experience of presenting to a range of audiences
  • Experience of health promotion
Skills
  • Highly developed skills relevant to the clinical area
  • Ability to demonstrate advanced level of clinical skills within the specialist field.
  • Coaching and training skills
  • Ability to communicate with healthcare professionals and the public including people with communication difficulties
  • Ability to motivate others
