Specialist OT Live Well Age Well

The Live Well team is recruiting a team leader to develop our existing Live Well Age Well project, supporting frail residents of Cardiff and the Vale to self‑manage and remain healthy in the community for longer. The role involves managing the Live Well Age Well programme and supporting the multidisciplinary team (MDT) to deliver high‑value, high‑impact interventions. You will work collaboratively with community partners across the health board’s geographic area and manage a support staff team delivering this preventative programme.
The ability to speak Welsh is desirable;

- Manage the Live Well Age Well programme and coordinate delivery of high‑value interventions.
- Collaborate with CRTs and community partners across the health board’s geographic area.
- Lead and support a team of staff to deliver preventative programmes.
- Support the MDT in planning and delivering services.
- Recognised qualification degree in Occupational Therapy.
- HCPC registration.
- Evidence of Masters level learning or relevant higher‑level qualification and experience.
- Membership of the professional body.
- Masters level qualification or equivalent.
- Significant post‑registration clinical experience in frailty, falls and healthy ageing.
- Evidence of clinical leadership and involvement in planning and developing an existing service.
- Experience working as part of a multidisciplinary team.
- Experience teaching other disciplines.
- NHS experience.
- Experience working with early signs of cognitive decline.
Provided advice and support to other practitioners to develop skills and knowledge. - Involvement in developing professional practice and transforming service delivery.
Experience presenting to a range of audiences. - Experience in health promotion.
- Highly developed clinical skills relevant to the occupational therapy area.
- Advanced level of clinical skills within the specialist field.
- Coaching and training skills.
- Effective communication with healthcare professionals and the public, including those with communication difficulties.
- Motivational skills and ability to inspire others.
- Quality improvement and research methodology.
- Specialist skills within clinical area.
- Welsh language proficiency.
- Specialist knowledge in the clinical area.
- Evidence of relevant postgraduate training courses.
- Evidence of relevant C.P.D.
- Awareness of health and safety/clinical risk management.
- Professional confidence.
- Ability to cope under pressure and demonstrate resilience.
- Proactive management of own wellbeing.
- Enthusiastic, reliable, and a role model of UHB behaviours and values.
- Empathy, friendliness and approachability.
- Interest or skill set in service development and planning.
- Ability to travel in a timely manner, including cross‑site work and various locations.
- Desirable Welsh language ability.
- Membership of relevant special interest group.
