Position Summary:

We are seeking a dedicated and patient-focused Licensed Practical Nurse (LPN) to join our clinical team. The ideal candidate will support our providers in delivering high-quality care while ensuring an efficient, friendly, and professional patient experience. • Room patients, obtain vital signs, and review/update medical histories
• Assist with procedures and examinations under the supervision of our providers
• Administer medications and injections as ordered
• Provide patient education and follow-up instructions
• Document clinical information accurately in the electronic health record (EHR)
• Coordinate with care teams and assist with care management tasks
• Perform phlebotomy and lab specimen collection (if certified/trained)
• Maintain exam rooms, medical equipment, and inventory of supplies
• Support telehealth visits and remote patient monitoring as needed

• Current and unrestricted Illinois LPN license required
• Previous experience in a primary care or outpatient clinic preferred
• CPR/BLS certification required
• Strong communication and interpersonal skills
• Detail-oriented with strong organizational and documentation abilities
• EMR/EHR experience preferred (we use Epic)
