The Transitional Housing Site Manager/Case Manager is responsible for the daily operations of the YHDP-funded transitional housing program, ensuring a safe, stable, and supportive living environment for youth experiencing homelessness. This role combines property management with case management services, working directly with young adults (ages 16-24) to support their transition to permanent housing, employment, education, and self-sufficiency.

Key Responsibilities:
  • Oversee the day-to-day operations of the transitional housing facility, ensuring a safe and well-maintained environment.
  • Enforce program policies, house rules, and participant agreements while maintaining a trauma-informed and youth-centered approach.
  • Conduct unit inspections and coordinate maintenance requests, ensuring compliance with local housing codes and safety regulations.
  • Maintain accurate records of unit occupancy, lease agreements (if applicable), and incident reports.
  • Ensure that participants understand their rights and responsibilities within the program.
Case Management & Supportive Services:
  • Provide individualized case management services, including housing stability planning, life skills development, education and employment assistance, and referrals to supportive services.
  • Conduct comprehensive intake assessments and develop individualized service plans with program participants.
  • Support participants in securing permanent housing by assisting with applications, landlord engagement, and housing navigation.
  • Connect youth to community-based resources such as mental health services, substance use treatment, healthcare, education, and employment programs.
  • Facilitate life skills workshops, including budgeting, tenant rights and responsibilities, conflict resolution, and job readiness.
  • Maintain case notes and documentation in the agency’s case management system, ensuring compliance with grant requirements.
Crisis Intervention & Youth Engagement:
  • Provide crisis intervention, conflict resolution, and mediation as needed to support a positive living environment.
  • Foster a supportive and empowering space where youth feel safe, heard, and respected.
  • Collaborate with youth to develop and achieve personal goals, advocating for their needs within the system of care.
  • Promote youth voice and leadership within the program, incorporating feedback to enhance service delivery.
Program Compliance & Reporting:
  • Ensure compliance with YHDP, HUD, and agency policies, including Housing First, Positive Youth Development, and Trauma-Informed Care principles.
  • Track and report program outcomes, including housing stability, employment, and educational progress of participants.
  • Participate in Coordinated Entry and case conferencing meetings to ensure seamless service coordination.
  • Assist with grant reporting, data entry, and required documentation for funders and oversight bodies.
Qualifications & Skills:
  • Education:

    • Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred. Equivalent experience in youth services, housing programs, or case management may be considered.
  • Experience:

    • Minimum of 2 years working with youth experiencing homelessness, transitional housing, or related social services.
  • Knowledge:

    • Understanding of trauma-informed care, Housing First, Positive Youth Development, and harm reduction approaches.
  • Skills:
    • Strong case management and advocacy skills.
    • Ability to de-escalate crises and provide conflict resolution.
    • Strong organizational and documentation skills.
    • Excellent interpersonal and communication skills to engage youth effectively.
    • Experience with property management, housing navigation, or landlord relations is a plus.
  • Technology:

    • Proficiency in case management software (such as Apricot), Microsoft Office, and online databases.
  • Other Requirements:
    • Valid driver’s license and reliable transportation.
    • Ability to work flexible hours, including occasional evenings and weekends.
    • Ability to pass background checks in accordance with agency policies.
Work Environment:
  • Work will take place in both office and residential settings.
  • Regular travel may be required to housing sites, partner agencies, and community meetings.
  • Requires the ability to remain calm and professional in high‑stress situations involving youth in crisis.
