Salary Range: $76,000 – $79,000 annually (DOE)

As a Hospice RN Case Manager

• Provide skilled, compassionate nursing care in patients’ homes and care facilities.
• Complete comprehensive assessments and develop individualized plans of care that honor patient goals and dignity.
• Educate and support patients, families, and caregivers with clarity and empathy.
• Collaborate closely with our interdisciplinary team to ensure seamless, holistic care.
• Manage medications, treatments, and symptom control with excellence and compassion.
• Participate in an on-call rotation to ensure continuous, dignified care for our patients.

• Active Registered Nurse (RN) license (required).
• Hospice, palliative, home health, or geriatric nursing experience (preferred).
• Strong clinical judgment paired with a calm, reassuring presence.
• Ability to work independently while thriving as part of a collaborative team.
• Excellent communication, organization, and documentation skills.
