Primary Shift: M-F, 8:00 am – 5:00 pm (some evening hours as needed)

Salary: $37,500 - $40,000/annually

Catholic Charities of Southern Missouri is seeking a caring and committed Care Manager to join our Maternal and Family Programs team in Cape Girardeau, serving both the Life House residential program and Healthy Moms, Healthy Babies. Life House provides a safe, nurturing home for pregnant women experiencing homelessness, while Healthy Moms, Healthy Babies offers community-based pregnancy and parenting education that supports healthy pregnancies, confident parenting, and long-term stability.

In this mission-driven role, you will use a strengths-based, trauma-informed approach to empower clients, develop individualized care plans, and remove barriers to self-sufficiency, while collaborating with a multidisciplinary team and community partners across housing, healthcare, education, and employment systems. What You’ll Do
  • Independently assess client strengths, needs, risks, and barriers by conducting comprehensive evaluations using professional judgment, evidence-based practices, and trauma-informed principles to inform service planning and intervention strategies.
  • Develop, implement, monitor, and regularly revise individualized care plans that address housing stability, health care access, education, employment, parenting, and self-sufficiency goals, exercising discretion to tailor plans based on client progress, compliance, and changing circumstances.
  • Provide ongoing professional case management to residential and aftercare clients, including conducting regular in-person and virtual meetings, evaluating progress toward established goals, and determining appropriate service adjustments to support long-term stability.
  • Coordinate and integrate services across multiple systems, including healthcare providers, mental health professionals, housing agencies, educational institutions, legal resources, and community partners, ensuring services are aligned, effective, and consistent with client needs and program requirements.
  • Serve as a professional advocate for clients by representing their needs and interests with external service providers and community organizations, applying sound judgment to resolve service gaps, address barriers, and promote continuity of care.
  • Apply trauma-informed care, motivational interviewing, and strengths-based approaches in all client interactions, selecting and adapting engagement strategies based on professional assessment of individual readiness, risk factors, and therapeutic needs.
  • Evaluate client compliance with program agreements and expectations and, using professional discretion, develop and implement corrective, supportive, or graduated intervention plans to address noncompliance while supporting client accountability and progress.
  • Oversee and coordinate client participation in classes, groups, and educational programming, exercising judgment in determining appropriate referrals, participation levels, and sequencing of services in collaboration with internal staff and external partners.
  • Maintain responsibility for accurate, timely, and compliant case documentation, reports, and data entry in accordance with agency policies, funder requirements, and professional standards, ensuring documentation reflects professional assessments, decision-making, and outcomes.
