Management
Reports to Executive Director of Integrated Health and Social Care, with matrixed reporting to the local Mission Leader. Supports development of periodic Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP). Assists in developing and implementing key initiatives to successfully achieve goals of the CHIP. Represents Mercy in local collaborative efforts with community agencies and partners. Coordinates local community benefit data collection and reporting.
Monitors and tracks data and metrics that demonstrate the impact of community health initiatives. Oversees local Community Health Worker program, directly supervising CHWs in ED, clinic, and hospital. Manages and/or provides technical assistance of local grants (as needed) for community health services and patient assistance as applicable.
- Education:
Bachelor's degree in public or community health, healthcare, health education or related field required. - Experience:
2-3 years' experience in community health or related field. - Preferred Education:
Master of Public Health (MPH) or Master of Healthcare Administration (MHA) preferred. - Other:
Must understand English both written and spoken, must be able to safely and successfully perform job-related functions, with or without reasonable accommodations required by federal, state, or local law, must be able to travel between Mercy communities.
