Lee Physician Group is seeking an experienced RN to join their Adult Rheumatology clinics. The ideal RN will have outpatient and/or rheumatology experience and a minimum of 3 years of nursing experience. This position is primarily located at the Surfside Outpatient Clinic; occasional travel to Fort Myers and Bonita for coverage may be required.

The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, promote, and maintain optimal health status of assigned patients. The coordinator functions in partnership with the patient, physician, and care team in the office setting, serving in an advocacy role to ensure delivery of quality, efficient, and cost‑effective patient‑centric health care services. The case load consists of moderate to high risk patients.

• Minimum 3 years of clinical nursing experience with emphasis on patient education and chronic disease management (home health, skilled nursing, medical‑surgical). Care coordination experience preferred.
• Bachelor’s degree in Nursing preferred; associate degree in Nursing required.
• Florida Registered Nurse license required.
• Travel to Fort Myers and Bonita may be required.
• Other state licensure requirements are not applicable.
