HOME CARE - RN CASE MANAGER

The Aultman Registered Nurse (RN) provides direct, primary nursing care to patients/residents. The RN delegates and coordinates patient care based on a documented assessment of the patient’s needs. The RN supervises the care provided by licensed practical nurses, patient care technicians, state‑tested nursing assistants, medical assistants, and others as applicable.

• Graduate of an accredited school of nursing; BSN preferred.
• Current, active licensure as a Registered Nurse through the Ohio Board of Nursing.
• Maintain current, active BLS certification through the American Heart Association.
• Certifications preferred for RNs meeting applicable eligibility requirements.
• Membership in a professional nursing organization preferred.
• One year nursing experience preferred; home health experience desirable.

• Apply the nursing process of assessment, planning, and implementation.
• Serve as a patient advocate and promote safety, comfort, and satisfaction.
• Maintain knowledge and competency reflecting current nursing practice.
• Collaborate with patients, families, and the healthcare team to facilitate care.
• Lead quality improvement initiatives and implement HRO tools.
• Ensure timely communication of patient information.
• Review emails, department communications, and learning assignments promptly.
• Perform continuous care review for every patient on the caseload (average 20–25 patients).
• Complete initial and comprehensive assessments to determine home‑health care needs.
• Obtain orders and referrals as needed, and complete OASIS and other assessments.
• Develop and update plans of care, incorporate therapeutic actions, and involve families in goal setting.
• Provide education and counseling to patients and caregivers.
• Document all observations, interventions, and communications within agency time frames.
• Coordinate equipment, supplies, and community resources.
• Participate in case conferences, QAPI, and infection control programs.
• Ensure compliance with Medicare, Medicaid, and HIPAA regulations.
• Meet daily and weekly productivity standards and complete annual competency requirements.
• Support the organization’s strategic goals and adhere to the Aultman Code of Conduct.

• Effective communication and professional interaction with colleagues, physicians, patients, and visitors.
• Work independently and as part of a team.

• Shift schedules may vary with staffing needs.
• Visits may be required during nighttime, evening, or inclement weather.
• Work weekends and holidays as assigned.
• Participate in on‑call rotations.
• Travel within the agency’s service areas.
• Frequent interruptions and priority changes during shifts.

Includes lifting (0–10 lbs to over 100 lbs), forward reaching, overhead reaching, standing, walking, sitting, climbing stairs/ladders, stooping, pushing, pulling, and repetitive grasper/sustained fine motor tasks. Vision (near/far) and hearing (ordinary) are required.
