Position: TEAM ASSISTANT - HOME CARE

Purpose of Position

The purpose of the Team Assistant position is to process and coordinate all required documentation and communication, both paper and electronic, to facilitate complete, accurate, and timely flow of patient information. This includes, but is not limited to, assistance with orders management, ongoing authorizations (private insurance) tracking, pre-bill, and general clerical support to the clinical team.

Responsibilities & Expectations
  • Gathers needed information for the proper intake of patients into the Matrix Care system.
  • Communicates with appropriate corporate insurance departments regarding authorizations.
  • Verifies home health care benefits according to payor plans
  • Verifies following provider for all patients and obtains patient records and current medication list
  • Coordinates, tracks, obtains and follows up on ongoing insurance authorizations as assigned
  • Ensures effective communication and collaboration with office and field staff in order to ensure continuity of care.
  • Assembles and maintains departmental and patient records.
  • Utilizes all applicable computerized systems within the department.
  • Runs reports as requested for Directors and Clinical Managers to monitor office operations.
  • Maintains up to date and complete medical records.
  • Assists the Referral Management department with daily follow up on hospitalized patients.
  • Consistently communicates with all patients, families, customers, and other personnel in a professional, respectful and courteous manner.
  • Complies with agency infection control and safety policies.
  • Completes annual education requirements.
  • Promotes teamwork by demonstrating willingness to assist others in meeting the needs of all individuals and collaborating effectively with the interdisciplinary team and other departments throughout the organization.
  • Demonstrates flexibility in meeting the needs of the program and responds positively to changes in workflow.
  • Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel.
  • Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by the Director.
  • Consistently maintains a professional appearance.
  • Participates in process improvement activities.
  • Protects patient health information according to the HIPAA Privacy Rule and Aultman policies.
  • Creates and fosters a work environment that is consistent with the mission, vision and values of the Aultman Health Foundation.
  • Supports the organizational strategic goals.
  • Abides by the Aultman Code of Conduct at all times.

Qualifications
  • High School Diploma with basic knowledge of healthcare and medical terminology, home health experience preferred
  • Basic word processing and data entry skills.
  • Able to work independently and accurately with attention to detail
  • Excellent interpersonal and communications skills; strong organizational and problem-solving skills, ability to stimulate new ideas.
  • Ability to maintain confidential information.
Working Conditions
  • Hours of operation with shifts as assigned.
  • Coordinates breaks and lunch time with others in the department
  • Subject to frequent interruptions and changes in priority of duties throughout the day.
  • Subject to emergency and other crisis situations
  • Hazardous Exposure Category 1
