Provide direct care and recreational services to individuals. Complies with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks.
Essential Duties and Responsibilities- Provides assistance with personal care including but not limited to bathing, showering, toileting, dressing, grooming and moving and transferring of individuals.
- Plan and supervise recreational activities in house and in the community with individuals.
- Take and record vital signs and perform assigned physical therapy exercises.
- Assist with feeding and monitoring food and liquid input and output.
- Provide educational training, counseling and physical assistance when necessary to achieve ISP goals.
- Observes patient, records significant conditions and reactions, and notifies Director, Nurse or Physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
- Attends meetings, in-services, significant medical appointments, and case conferences as necessary.
- Other duties and/or assignments to work/program locations based on the needs of the company.
This job has no supervisory responsibilities.

Education and/or ExperienceHigh school diploma or general education degree (GED); and nursing assistant certification.
Language SkillsAbility to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write and proofread routine reports and correspondence. Ability to speak and present effectively before individuals or employees of an organization.
Mathematical SkillsAbility to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning AbilityAbility to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Vehicle/TravelRequires a legally insured and inspected automobile for use to be presented at work and available for use during working hours. If the program has an agency vehicle, staff are expected to drive it as needed.
Certificates, Licenses, RegistrationsValid and current driver’s license from the state of residency, certified nursing assistant certification.
