Cherry Brook is a 100 bed skilled nursing facility in Canton, CT. Rated 5 stars, we're looking to add 16 hours per week of support to our MDS department. This position can be a nurse or non‑nurse, but would work in harmony with the two MDS nurses we have. Must have experience working in an MDS department.

About the Role:

As an MDS (Minimum Data Set) Staff Member Cherry Brook Health Care Center, you will play a crucial role in ensuring accurate and timely completion of resident assessments. Your main objective will be to gather and analyze data to develop comprehensive care plans that meet the individual needs of our residents. By accurately documenting and coding resident information, you will contribute to the overall quality of care provided at our facility.

Your attention to detail and ability to navigate complex regulations will be essential in maintaining compliance with state and federal guidelines. Ultimately, your work will directly impact the well‑being and quality of life for our residents.

Minimum Qualifications:

  • Strong knowledge of state and federal regulations related to MDS assessments.
  • Excellent attention to detail and organizational skills.
  • Effective communication and interpersonal skills.
  • Experience working in a long‑term care or skilled nursing facility.
  • Familiarity with electronic health record systems.
  • Experience with quality improvement initiatives.
