Job Summary:

The Physician provides comprehensive longitudinal primary health care with a strong emphasis on health promotion, disease prevention, equity, and community responsiveness. Practice is carried out in accordance with the standards, policies, and guidelines of the College of Physicians and Surgeons of Ontario (CPSO), the Regulated Health Professions Act, and applicable legislation.

The physician diagnoses and manages common complex health conditions independently and collaboratively, working as a core member of an interdisciplinary team. The Physician contributes to the planning, delivery, monitoring, and evaluation of primary care and community-based services, with particular attention to the social determinants of health, healthy equity, cultural safety, and trauma‑informed care.

The physician is professionally accountable for their clinical practice and contributes to continuous quality improvement, system integration, and the overall mission of the Community Health Centres of Northumberland (CHCN).

Service Responsibilities:

  • Provides on‑going, comprehensive primary health care to clients of all ages within the scope of a Family Physician, including assessments, diagnosis, counselling, screening, referrals, education, treatment, prescribing and follow‑ups.
  • Assesses each client to determine the presence of existing and potential health problems, with a major focus on the physiological, psychological, social‑cultural, spiritual and environmental risk factors that may influence the client’s health status. The Physician deals with these personally or by referral to, or in consultation with, other service providers and in the community.
  • Establishes a plan of care in collaboration with the client and encourages increased responsibility for health by involving clients and their caregivers in; a) the identification of risk factors and health problems, b) goal setting, c) decision making for interventions and treatment options, d) prevention and health promotion activities, e) preventative screening with emphasis on breast, colon and cervical cancer screening.
  • Makes internal referrals to other CHCN staff.
  • Participates in case conferences with other clinicians/providers to ensure coordination of services to the client.
  • Keeps complete, accurate and timely records of client visits, which will provide information to assist other practitioners in continuing client care.
  • Sees clients at the Centre with scheduled and urgent appointments, in the community through outreach and home visits, during regular and extended hours and shares on‑call duties with other primary health care providers.
  • Promotes awareness of the CHCN’s programs, community projects or agency activities. Participates in above as a representative of the CHCN.
  • Provides consultation to other providers and clinicians.
  • Performs necessary back‑up clinical functions: test results review, response to messages and prescription renewal for other clinical staff in their absence.
  • Provides support, teaching, and supervision of medical students who have placements at CHCN.
  • Participates in the development of, and gives support to, policies and strategies that are in the interest of community health and recognizes the social determinants of health.
  • Engages with vulnerable and marginalised populations and participates in active outreach across Northumberland County.
  • Participants in health and community fairs or events to promote health and wellbeing and CHCN programs and activities.
  • Works with community groups to support healthy public policy development and practice.
  • Incorporates health care advances into practice and bases this practice on the changing demographics and evolving needs of the community.
  • Strong commitment to adhering of screening guidelines and achieving benchmarks.
  • Completes monthly chart audits and data clean‑up activities.
