• Provide case management services for 50 HIV positive individuals.
• Provide emergent case management services for EIP patients as needed.
• Complete case management assessments and develop client service care plans.
• Monitor and evaluate service care plans quarterly.
• Assess all EIP clients requesting or referred for CM services, identify immediate/urgent needs, and take appropriate action.
• Meet objectives for the provision of referrals and linkages to support services.
• Provide HIV counseling and testing as needed.
• Ensure CM services comply with AACO and NJDHSS standards for HIV medical case management services.

• 1 year or more experience in case management or social services.
• Experience in the field of HIV preferred.
• Extensive knowledge of available financial and social service resources in southern New Jersey.
• Excellent interpersonal skills.

• Bachelor's Degree required.

AACO or NJ HIV Case Management Certification preferred.

Bilingual is a plus.
