Responsibilities for a social worker:

• Assists residents with emotional adjustment to facility.
• As directed, interview new residents and records, social history, and assessments.
• Completes the social services portion of the MDS, care planning, baseline, etc.
• Refers to psychological services as needed.
• Schedules and attends and participates in interdisciplinary care plan meetings (morning meetings, utilization review, etc.).
• Writes summaries and progress notes as directed.
• Assists in conducting a pre-discharge evaluation including but not limited to setting up visiting nursing services, resources, etc. in the community.
• Assists with discharge planning. Follows up after discharge.
• Conducts required cut letters.
• Writes discharge summaries as assigned.
• Develops a professional relationship with each resident and family.
• Educates residents/representatives on advanced directives.
• Adheres to all social services regulations.

Qualifications for a Social Worker:

• Minimum bachelor's degree in Social Work from an accredited college.
• Must obtain a CSW and/or LSW.
• One to three years of professional experience and/or training; or equivalent combination of education and experience.
