Job SummaryResponsible to plan and coordinate programs and activities that are varied and reflect the interests, values, and beliefs of the residents and provide life-enriching opportunities for residents and the community.
Qualifications- High School Diploma or GED preferred
- Preferably one or more years of experience in working with adults in a health care or social service setting.
- Possess a valid New Jersey driver license with a driving record acceptable to the insurance company.
- Plan and implement activities in the following categories: physical (exercise), social, recreational, intellectual/educational, creative, entertainment, religious, service/volunteering and special request of residents.
- When a resident is admitted to the Activity Program, interview that resident and complete the Therapeutic Recreational Initial Interview form.
- Develop an individualized plan of care for each resident.
- The plan of care is to be updated at least quarterly.
- Develop a written monthly activities schedule at least one month in advance.
- Conspicuously post the current monthly activities calendar of events.
- A copy of the monthly calendar is to be given to each resident.
- Promote activities in a way that piques resident's interest and addresses the individualized care plans.
- Facilitate socialization among residents.
- Invite and/or remind residents to participate in the planned activities.
- Plan and provide transportation for outside activity events and regularly scheduled group trips; ex's shopping & banking.
- Provide escort service as needed to ensure resident safety on outings.
- Ensure that all appropriate safety measures are carried out while transporting residents.
- Organize and maintain an emergency information book to be taken with staff on resident outings.
- Complete necessary paper work regarding vehicles, mileage, expenditures, maintenance and cleaning.
- Satisfactorily complete the driver-training course.
- Complete CPR and First aid training.
- Train, support and instruct other staff members how to conduct activities.
- Establish and maintain constructive working relationships with co-workers, residents, families and visitors.
- Promote team work in providing services to residents.
- Coordinate with the kitchen any snacks/foods needed for activities.
- Assist with planning and implementing marketing events that promote the facility to potential residents.
- Cleaning of work areas.
- Identify and requisition needed supplies for planned activities.
- Reports behavioral changes or any unusual incidents of residents to appropriate staff.
- Complete Incident reports when required
- Assist in the placement and utilization of volunteers in the program.
- Adhere to and carry out all policies and procedures.
- Maintain confidentiality of verbal and written information pertaining to residents, facility operations and personnel.
- Carry out other duties as assigned by administrator to ensure smooth operation of the facility and to meet residents' needs.
